Request for Public Records

Shoreline Fire Department is committed to providing full access to public records in accordance with the Public Records Act. The Department will provide requestors the fullest assistance and the most timely actions possible while protecting public records from damage and preventing excessive interference with other essential agency functions. RCW 42.56.100

Requests should be made in writing by letter or by completing the Request for Public Records Form. Public records requests can be submitted in person or by mail.

Emergency Medical Report Request

Patients: To request a copy of your emergency medical report, please provide photo identification and complete the authorization for release of medical information.

Attorneys and other agencies: To request a report on behalf of a client, submit a written request on letterhead along with authorization to release medical information signed by your client.

A clerical fee of $26.00 will apply for medical records in addition to other authorized charges.
